HOW TO RSVP FOR THE BANQUET:

Step 1:   Submit the RSVP using the form below this box.

• 1 RSVP per athlete. If multiple athletes are attending from the same family please fill it out for each athlete. Just put extra guests on one of the athletes.

Step 2:   Submit payment to Coach Swanson.

• Cash or check? Bring to Coach Swanson in room E-33.

  • ALL MONEY MUST BE IN A SEALED ENVELOPE OR BAGGIE WITH THE FIRST AND LAST NAME OF THE ATHLETE ON THE FRONT.
